    • Review of PCP (Person Centered Plan) to make updates / changes as indicated/needed with each assigned client to be submitted to Qualified
    • Coordination of client care between Psychiatrist, Psychologists, Counselors
    • Referrals and coordination with services (Supported Employment, Independent Housing, Transportation)
    • Update and maintain Consent for Releases
    • Submit medication request to Psychiatrist if needed between appointments
    • Submit updated client information for psychiatrist for each client appointment
    • Verbal and written engagement with PSR staff / supervisor for specific issues related to assigned and non-assigned case load clients.
    • Update client information as needed in client files
  • Write and submit daily notes for each PSR group taught for the day including specific detail on effectiveness for each client attending group.
  • Maintain CPR / First Aid certifications , Non-Violent Crisis Intervention training
  • Filing of weekly PSR group notes for each PSR client note folder.
  • Occasional Information Technology support to PSR co-workers.
